Autograph draft. Written in pencil on both sides of a small lined page torn from a ring notebook. The date appears to have been added or changed later and, although it is in Collip's hand, it is likely in error. The extract was not referred to as insulin in December 1921. The note may have been written in the spring of 1922 when Collip went to Indianapolis with Best to brief Lilly Laboratories on methods for making insulin.

Annotated: 'The procedure for extracting insulin was written by Dr. Collip and left in possession of his wife in case something should happen to him ... Mrs. Collip kept this sheet in an envelope, on which she wrote 'original Collip process for preparing insulin. Dec 22/21.''
This is the only known extant laboratory note written by Collip relating to his Toronto research.
